The bruised bone is found in the center of the carpal bone area, colloquially known as the wrist, located at the distal end of the radius and ulna bone. It articulates with the third metacarpal bone (middle finger) and forms a third carpometacarpal joint. The enlarged bone is the largest carpal bone in human hands. It presents, above, the part or round head, received into the concave formed by the scaphoid bone and the crescent moon; limited parts or neck; and below, the body. This bone is also found in many other mammals, and homologous with the "third distal carpal" of reptiles and amphibians.

Structure

The capitate is the largest carpal bone found in the hand. The capitals are found in the distal carpal bones sequence. The hood is located right next to the finger metacarpal on its distal surface, having a hamate on the ulnar and trapezoidal surfaces on the radial surface, and adjacent to the crescent and proximal scaphoid.

Surface

The superior i <3> round, smooth, and articulated with crescent bones.

The inferior surfaces are divided by two mountains into three aspects, for articulation with second, third, and fourth metacaraw bone, which for the third being the largest.

Shallow surface wide and rough.

The palmar surface is narrow, rounded, and rough, for the installation of ligaments and part of the adductor pollicis muscle.

The lateral surfaces articulate with the lower multilayer by the small side in the anterior inferior angle, behind which is a crude depression for the attachment of the interosseal ligament. On top of this is a deep groove, rough, forming part of the neck, and serving for ligament adhesion; it is constrained superiorly by a smooth, convex surface, for articulation with scaphoid bone.

The medial surface articulates with bony bone by a smooth, concave, long side, which occupies the posterior and superior parts; it is roughly in front, for the installation of interosseous ligaments.

Variations

The bone that varies articulates with metacarpal index finger. However, normal articulation is with the middle finger.

Ossification

The ossification process starts in 1 - 5 months.

Function

The carpal bone serves as a unit to provide the bone superstructure for the hand. They allow wrist movement from side to side (medial to lateral) as well as up and down (anterior to posterior). Dr. H. A. Harris wrote in The British Medical Journal in 1944 that "the strength of hand construction in a man is concentrated on the fingers, thumb, and forefinger and middle finger." Therefore, the size is larger to sustain the strength and pressure experienced by the middle finger.

Clinical interests

Capitate fracture accounts for 1.3% of all fractures of the wrist. The isolated fracture of the capitate comprises only 0.3% and is often non-displaced. This is because the capitate is at the center of the Carpal area and is therefore adequately protected. Capitation fracture occurs along with other Carpal bone fractures - Scaphoid.

Various mechanisms for capitate fractures have been postulated. Adler et al. describes three mechanisms - the first is direct trauma to the dorsal surface of the bone, the second falls on the palm of the hand with the wrist in the forced extension and the third falls on the strongly flexed hand; the second most frequent and the third most rare.

In the case of acute Capitate fracture where there is x-ray evidence of perfect alignment of fracture fragments, the attending physician will paralyze the wrist in plaster or mild Wrist Brace. After the cast is lifted, the patient initiates physiotherapy to regain the range of wrist joint movement and strength on the muscles involved.

If the x-rays indicate that the fragment of the Capitate fracture is not aligned, the operation is indicated. A surgeon can use a small compression screw or K-Wires to unite two pieces of bone. Headless compression screw has advantages over K-wire because it provides compression throughout the fracture site and allows early movement. It is possible that the ligaments between the Capitate and Skafoid bone are also injured, so this will be fixed at the same time.

Because the Capitate has a poor blood supply there are sometimes complications with the healing process. It can manifest itself as a diffuse pain in the wrists on activity, and can last for months. This is caused by a disorder of the Capitate caused by a lack of blood supply and healing (Avascular Necrosis). Nonunion has been reported as the most common complication; 19.6% to 56% in isolated capitation fractures. Early diagnosis is the key to preventing this.
