5 Common Causes of Hip Pain in Women | Everyday Health

Pain in the hip is the experience of muscle or joint pain in the hip/pelvic area, a condition that usually arises from a number of factors. Sometimes it is closely related to lower back pain.

Common etiologies include:

  • Trochanteric bursitis, caused by an inflammation of the trochanteric bursa in the outer hip, often affects both hips
  • Pelvic arthritis, hip joint degeneration from osteonecrosis, trauma, sepsis, rheumatoid arthritis, or anatomical anomomy
  • Meralgia paresthetica, a chronic neurological disorder of the lateral femoral cutaneous nerve, most common among those who are pregnant or have diabetes
  • Hip avascular necrosis, bone tissue cell death in the hip joint brought by vascular occlusion or coagulation that results from old age, alcoholism, trauma, decompression disease, or some other possible cause; treatment of total hip replacement
  • Occult pelvic fractures, subtle cracks somewhere in the hip socket, common in elderly women and osteoporosis patients, usually in only one pelvis
  • Snap of the hip, a condition caused by an iliotibial snap band, iliopsoas tendon tendon, and a pelvic hip tear, usually on only one hip; can be accompanied by a "snap" that can be heard when the hip joint is moved
  • Paget's disease, enlarged or defective hip bone, genetic disorder; pain usually in both hips simultaneously
  • Malignancy, because pelvic cancer or proximal thigh bone can cause pain; usually only one affected hip
  • Primary septic arthritis caused by infection in the hip synovial fluid, a rare condition in adults except for those already immunocompromised as well as those with artificial hips; only one side of the affected pelvis, rapid onset of pain
  • Transient or accented synovitis or "irritability," the most common condition in children, more often in boys than girls, and disappearing alone within 7-10 days; pain only on one side
  • Sciatica, the condition most commonly caused by damage to the nerve roots of L4 or L5 but sometimes caused by inflammation or tension in the pelvic piriformis muscle (which rests on the sciatic nerve), in this case the condition is called piriformis syndrome; pain usually only occurs on one side but can occur on the other side at other times or (rarely) both sides simultaneously
  • Sacroiliac joint dysfunction, an unusual neurological condition in the largely immobilized sacroiliac joints caused by previous trauma to joints such as car accidents; pain usually only on one side
  • Radiculopathy, a nerve disorder caused by pressure or irritation of the nerves at the root (ie, near the spine) is often caused by spinal disc degeneration, joint degeneration, or osteoarthritis, among other causes

Pain in the groin, called anterior hip pain , is most often the result of osteoarthritis, osteonecrosis, concealed fractures, acute synovitis, and septic arthritis; pain in the side of the hip, called lateral pelvic pain , usually caused by bursitis; pain in the buttock, called posterior or gluteal hip pain , which is the most common type of hip pain, most commonly caused by sacroiliac joint dysfunction and sciatica (whether from spinal disks with bleeding or tensed piriformis muscle). Shingles can also cause posterior hip pain.
